Dr. Horvitz started practicing upon completion of his Osteopathic Family Practice residency in 1994. Dr. Horvitz has been in solo private family practice since 1998.

3.    Years in practice as a direct pay Wellness Concierge-lite primary care physician?

Dr. Horvitz founded the Institute for Medical Wellness, a Direct-Pay Blend of Traditional and Functional Wellness and Prevention practice in 2008.

4.    Do you have multiple locations?

.

The Institute for Medical Wellness has one location only at 110 Marter Avenue, Suite 408, Moorestown, NJ. The office is located in the Moorestown Office Center just off of Rte 38 and less than a mile from Exit 40 of Rte 295. By having only one location, Dr. Horvitz is not split between offices and is more easily available.

6.    What is your annual fee?

.

Fees vary dependent upon age and size of family. Please refer here, for more information.

7.    How Do I Pay The Doctor? Do you accept quarterly or monthly fees?

.

We accept fees paid annually or via an automatic monthly bank debit. Cash, Visa and Mastercard are accepted. Please refer here, for more information.

8.    Do you accept Insurance? How compatible is your direct-pay Wellness Concierge-lite prevention and wellness practice services with my health insurance plan?

.

The IMW has not participated with insurance plans since 2008. However your insurance is still used for most laboratory services and all services performed outside of our office as applicable to your insurance policy. Dr. Horvitz is considered an out of network physician for insurance purposes. Many of our patients and families have high deductible insurance plans and end up paying the same or less for care through our office. Flexible Spending Accounts (FSA’s) and Health Savings Accounts (HSA’s) are also popular with many of our patients. Please check with your healthcare/financial advisor if you can use HSA/FSA.

Dr. Horvitz and the IMW do participate in the R-Health program. More information on this program can be found here.

9.    Does Dr. Horvitz participate in Medicare?

.

The IMW Opted out of Medicare participation in July 2012. Medicare patients are welcome to receive care at our office by signing a Medicare private contract. By opting out of Medicare Dr. Horvitz is able to provide care outside of all the burdensome third party rules and regulations Medicare imposes. By opting out of Medicare and all other insurers, the IMW has no obligation to share your private medical records with any third party. Privacy is extremely important at the IMW!

10.    Do I need insurance to obtain treatment or to enroll or sign-up for an IMW Wellness Plan?

.

Our office does not participate with insurance, so insurance is not necessary to receive care from Dr. Horvitz. But health insurance is still suggested for use with all care outside of the IMW including specialty care, emergency care, hospitalization and other potential catastrophic costs.

11.    Is there a co-pay?

.

There are no co-pays if you are enrolled in an IMW Wellness Plan.

12.    What additional fees are not included in the annual Wellness Plan fee?

.

The annual Wellness Plan fee includes prepayment for your care from Dr. Horvitz at the IMW. Additional fees may be charged for housecalls, immunizations, supplements, legal work, copying records, some laboratory tests and other Special Testing not covered under your health insurance plan. Dr. Horvitz will do his best to advise you in advance if he know

13.    Will I be required to pay even if I do not need to use your services?

.

The IMW Wellness Plan has a minimum one year commitment regardless of the amount of service you should need.

14.    What services can I expect to receive each year during my membership with your practice?

.

One annual IMW Wellness Exam focusing on wellness, prevention and improving your health.

IMW Advanced Wellness Testing- Most of the cost of the testing is charged thru your health insurance.

Same and Next day appointments for sick visits with very little waiting.

No access-phobia about communicating with Dr. Horvitz. Whether in office, by phone, secure patient portal, Spruce texting and video, Dr. Horvitz is available.

Individualized care with a blend of traditional/functional/holistic approach.

15.    May I see a copy of your Medical Service(s) Agreement?

.

All IMW Wellness plan members receive a copy of the Medical Service Agreement.

16.    What If I Need To Go To The Hospital or Emergency Room?

.

The IMW performs all of its services outside of the hospital or emergency room. Our job is to hopefully keep you from needing those services. But if you do go to the hospital or emergency room, your health insurance will used as applicable. We do ask for you to inform the hospital to notify our office that you were seen or admitted so we can discuss your case with the hospital physician in charge. Also, if you are not sure whether you need urgent or emergent care, we advise you to contact Dr. Horvitz so he can triage your care. Dr. Horvitz may be able to save you an unneeded trip to the hospital or urgent care center.

17.    Will my insurance reimburse my monthly fee?

.

The IMW does not participate with insurance plans. Reimbursement is between your insurance plan and you. Dr.Horvitz is considered an out-of-network physician with insurers and you should read your insurance contract for how that is handled. For further discussion please ask Dr. Horvitz how other patients have handled this.

18.    Does Dr. Horvitz  bill Medicare for the monthly retainer fee?

.

No, the IMW opted-out of Medicare on July 1, 2012. The IMW does not send any bills or correspondence to Medicare. We also do not send any of you medical records to Medicare, so your records are truly private in our office.

19.    Tell me about what’s included in the Annual IMW Wellness Exam.

.

One annual IMW Wellness Exam is included with your plan. This exam focuses on your individual health and wellness and is different and I believe superior to most annual exams. The IMW Wellness exam takes place over a few visits and can last a combined two hours or more. A very thorough medical and family history is obtained or reviewed to determine your present and future risks. A complete head to toe physical exam focusing more on keeping you independent and functional is the goal, not just following rote guidelines. Dr. Horvitz uses medical guidelines as a rough draft, and helps you decide whether the guidelines are appropriate for you as an individual. Individualized laboratory testing including the IMW Advanced Wellness Testing (AWT) is advised for all patients and other Special testing is conducted and then reviewed when available. The ultimate goal of the IMW Wellness Exam is to show you not only where you are healthy, but also where you need to make some changes. The most important part of the IMW Wellness Exam is the review of findings, both physical and testing, and then the planning that helps you improve in areas that need work. Our goal at the IMW is to keep you healthy and hopefully achieve optimal health.

20.    Tell me about after-hours care and the doctors 24/7 Availability.

.

Dr. Horvitz is available by phone for after-hours care. Dr. Horvitz is also available for non-urgent issues by HIPPA secured patient portal and Spruce.

21.    Can I still see my gynecologist?

.

Yes. Dr. Horvitz does not perform routine gynecology. If you need a gynecologist, Dr. Horvitz can refer you to one in the area.

.

22.    What is your annual patient retention? (i.e. do you retain most of your patients annually?)

.

The annual retention rate has been at least 90% yearly since 2008. Of the 10% who leave, over half have been due to relocation. Unlike traditional concierge practices that limit practice size to an average of 500 patients, Dr. Horvitz and the IMW has set a limit on practice size to 1000. If and when Dr. Horvitz can not accommodate all his patients on a same or next day basis, at that point he will consider closing to new patients. As of July 1, 2019, the IMW welcomes new patients.

23.    What about lab, x-ray and specialists’ fees and hospitalizations?

.

You would use your health insurance as applicable.

24.    How long do in-office visits typically last?

.

In office visits can last from 20 minutes to over an hour, dependent upon the need. However if you are in a rush, they can be shorter ; )

25.    How long will I have to wait if I come in for a visit?

.

Waiting times average less than five minutes, if there is a wait.

.

26.    What if I have an emergency or need hospitalization?

.

Emergencies of hospitalizations need “hospital level care”. We hope that you do not need this level of care, but do ask that you notify our office, if able, prior to visiting a hospital for care. We also ask you to inform the hospital to notify our office that you were seen or admitted so we can discuss your case with the hospital physician in charge. Even if we are not the treating hospital physician, we can still be involved as an advocate for your care.

27.    What hospitals does Dr. Horvitz have admitting privileges to in this area?

.

Dr. Horvitz and the IMW focuses on your overall Wellness. We do our best to teach you how to avoid getting sick and how to improve your health issues. Thus, our job is to do our best to keep you from needing hospital level care. If you do need hospital level care, Dr. Horvitz may recommend a hospital or specialist to care for you while in the hospital.

28.    What about the cost of prescription medicines?

.

Prescription medications are not included as part of the IMW Wellness Plan. But Dr. Horvitz is very cost conscious and will discuss with you the costs of most tests and medications. If you feel that a medication or treatment is too expensive, please talk to Dr. Horvitz to see if a less expensive alternative is available.

29.    Can I receive text messages from my doctor? Secure E-visits via video chat, or thru an iPhone app?

.

Yes! Dr. Horvitz uses both Elation Patient Portal and the Spruce app for this type of communication.

30.    What type of new technology do you use regularly in your direct-pay Wellness Concierge-lite prevention and wellness medical practice?

.

The IMW takes advantage of internet and social media where it is secure and controlled. Our website, www.drhorvitz.com contains information about our practice as well as hosting Dr. Horvitz’s blog. 31.    If I call Dr. Horvitz at 8pm or 2 am for an urgent healthcare concern, what will happen? I.e. Will Dr. Horvitz be available?

 Dr. Horvitz is available by phone 24/7 for urgent healthcare concerns.

32.    What if I need to see a medical specialist or need surgery?

.

Dr. Horvitz has been practicing in the Moorestown, NJ area since 1998 and South Jersey since 1994. If you need a specialist or surgery, Dr. Horvitz will help you find the appropriate physician, healthcare professional or hospital.

33.    How difficult is it to get an appointment? Do you provide Same Or Next-Day Visits With Unhurried Appointments that start On-Time with the doctor?

.

The IMW offers same or next-day appointments as standard policy. If you call for a sick/urgent visit in the morning on a day the office is open, you will be offered a same or next day appointment, usually same day. The waiting time if any is usually less than five minutes. Please see our mission statement.

34.    What happens if Dr. Horvitz goes on vacation?

.

If Dr. Horvitz is on vacation, he will usually have a few doctors on call. The Spruce app will help you get in touch with the IMW to help triage your need for medical care. Dr. Horvitz’s medical assistant will also be available to triage your concerns and help you obtain the care you need.

35.    What happens if I move out of the area after I enroll?

.

All IMW Wellness Plans have a one year minimum. You can still obtain wellness and preventive care via telephone, or or other secure means of communication such as the patient portal and Spruce. The IMW has many Wellness Plan members who live outside of the area who remain members as the Wellness aspects of care are so unique.

36.    Can I enroll in an IMW Wellness Plan anytime in the future?

.

It is your decision when to enroll in an IMW Wellness Plan. But why wait??

37.    What if I get married or have/adopt a child?  Can I extend coverage to my new family members?

.

The IMW has individual, couple, family and corporate plans available. Dr. Horvitz limits his practice to individuals 12 years of age and older, and children who have finished their initial set of immunizations.

38.    Do you have any specials/discounts running right now?

The specials we always have running are for couples and families. The fee structure rises slightly when adding a family member, but is much less than if they joined separately. See our payment options section for more details.

40.    How do I sign up for an IMW Wellness Plan?

.

All potential new patients are highly encouraged to schedule a Free-No-Obligation 15 minute Meet-N-Greet with Dr. Horvitz prior to joining the practice. This 15 minute meeting gives you the time to ask any other questions you may have about Dr. Horvitz prior to joining or signing any membership contracts. The IMW is built on building and maintaining strong healthy relationships, starting from our very first meeting. To sign up just stop by our office during our regular hours, review the medical services agreement, decide upon whether your fee will be paid all at once or via automatic monthly debit plan. Then get ready for some Prevention, Wellness, and healthcare the IMW way!!
