There are No Copays with the Wellness plans. One yearly Wellness physical per year is included. The only other fees are for selected injections, nutritional products, housecalls, immunizations, and/or non-medical issues. Dr. Horvitz set up Wellness Plans that do not reward him for performing extra procedures, tests, and office visits. The Wellness Plans were set up to put a ceiling on your medical expenses at our office, while offering the absolute best in care, without compromise!

Fees for The Optimal Wellness Plan are listed below:  (As of November 1, 2011)

Individuals Ages 6-24:             $100 down and $45 a month for one year or a once yearly fee of $499. 
                                             

Individuals Ages 25 to 29:        $100 down and $50 a month for one year or a once yearly fee of $549. 
                                            

Individuals Ages 30 and up:      $100 down and $55 a month for one year or a once yearly fee of $649.
                                             

Married couples:                      $150 down and $90 a month for one year or a once yearly fee of $1099.
                                             

Family Plan:                            $200 down and $110 a month for one year or a once yearly fee of $1299.

The Traditional Wellness Plan will be phased out at the end of 2011 and replaced with the Optimal Wellness Plan.
